1. ARC card, passport or R.O.C. National ID Card;
2. The applicant's household registration book or a transcript of the current household within the last three months.

3. For applicants who are low-income household, low-middle-income household or a family with special circumstances, please provide the current year’s income or special circumstance certificate.

4. If the documents listed in No.3 above are unavailable to provide, please provide a list of property ownership information for the entire household (applicant, parents, children and other relatives living with the same property), and a list of various types of income from comprehensive income tax in the most recent year (please contact the IRS: Tel: 0800-000-321)
