Required Documents:

1.NHI Application Form for Refund of Out-of-pocket Medical Expenses.

2.Need provide original copies of your medical expense receipts and breakdown.

If you accidently lost the original copies or already used them for other purposes, you can apply for a duplicate copy at former medical institutions, and note to have it stamped with the hospital chops plus another stamp stating「same with original」. While no need stamps for duplicate copies of treatments done abroad, if difficult to do so. In addition, there is a need to provide「Declaration for No Original Medical Expenses Receipts」and need state the reason why, if submitted is not original, no matter where the treatment is done.

3.Please obtain following documents from the physician or hospital, if diagnosis certificate or supporting documents of treatment is from non-NHI contracted clinics/hospitals, or outside the NHI implementation region (including China and other foreign countries):

(1) For outpatient checkup or emergency treatment, please prepare「Diagnosis Certificate」 (content should specify the symptoms and name of the diagnosed disease).

(2) For confined hospitalization, need further obtain a「Hospital Discharge Summary」aside from the「Diagnosis Certificate」(again, content must specify symptoms and diagnosed disease name).

4.If refund application is for out-of-pocket medical expenses incurred overseas, please prepare  document copies of time proof for your entry and exit (like passport copies with photo attached and stamps of Taiwan exit and entry), Certificate of Entry and Exit Dates applied online and issued by the National Immigration Agency, or proofs issued by relevant departments. If you pass thru the「Automatic Customs Inspection System」during customs clearance, you can find stamps of entry and exit at the「Official inspection counter」near exit.  Alternatively, you can get access to the website of National Immigration Agency, Ministry of the Interior, find area named「Departure/Arrival Date Certificate Online Application System」online and print out certificates, or simply just indicate「Use of automatic custom inspection clearance」as remark on first page of passport copy, and on the attachment area of application form. Those who have not yet entered Taiwan, can appoint others to apply, but need attach “Authorization Letter” during application.

5. In the case of whole-year cumulative expense of hospitalization exceeding statutory amount limit, no need to submit original receipt and expense details during application, when one agrees for NHI Administration to calculate the refund amount based on contracted hospitals’ given information.

How to Apply

Please collect all documents specified above and file application online via global information website of NHI Administration (but still need to send the actual application form, original receipts, diagnosis certificate, and hospital discharge summary by registered mail), file by sending mails or apply in person at NHI counter:

1. To apply for refund of expenses incurred overseas, please file application at the regional division of NHI Administration where you are insured.

2. To apply for refund of expenses incurred within Taiwan, please file application at the regional division offices of NHI Administration (with location relevant to where you have taken medical treatments), or simply submit it to the regional offices of NHI Administration with location relevant to your residency, and they will help forward and file application to the relative department.
