Confirmation of employer (applicant) and caregiver qualification
Employer (applicant) and ward should have one of the following relationships
2.Direct Relative by Blood
3.Third - Degree relatives by blood.
4.First Degree Relative by marriage
5.Grandparents and grandchildren-in law
Ward who wants to hire a family nursing worker should have one of the following conditions:
(1)The ward being cared for is provided with a certificate of physical and mental impairment issued by the municipal or county (city) social administration authority and must comply with one of the “Severe or Very Serious condition of Specific Physical and Mental Disorders” in the table below.

(2)The ward shall have the professional assessment of the “Applicant for the Employment of Foreign Family Care Workers Evaluation Hospital” designated by the announcement, and the medical certificate shall be considered by the designated hospital for 24 hours. The assessment results are as one of the following:

Remarks: To apply for a family nursing worker, need an assessment from the hospital, or access directly to the website of the Ministry of Health and Welfare
(3)Ward who meets the following condition are exempt from medical evaluation (only applicable for applying a rehire foreign worker)
Since March 5, 2015, those who have been in compliance with the following three types of conditions can be exempted from professional evaluation of medical institutions, namely that they are eligible to apply for a family nursing worker.
First Category: Ward is over 80 years old and under 85 years old, and had professional evaluation of the medical institution and being determined that there is a serious dependence or need a full-time care , or the ward is over 85 years old, and has been assessed to be mild, heavily dependent or need a full-time care by the medical institution, can apply for re-recruitment of family nursing workers.
Second Category: Ward has been assessed by the medical professional to have full-time care needs, and is dysfunctional from cerebral palsy, life impairment from spinal cord injury, amputation or other symptoms, can apply for re-recruitment of a foreign family nursing workers.
Third Category: Ward undergo a professional medical evaluation and has determined that there is a need for full-time care, and cannot get out of bed certified by the medical institution. Ward needs 24 hours use of respiratory machine or life-support equipment, and vegetative certificate can apply for re-recruitment of a foreign family nursing worker.
Filling up [Application for Employment of Foreign family nursing worker Basic Data Transfer Form] to Long Term Service Center
Applicants should confirm whether they are eligible to "Employer (Applicant) Qualification" and "Ward Qualification" and proceed with the following methods after the confirmation. If the employer has any questions, please contact the county and city government's long-term care management center. (hereinafter referred to as the long-term Service Center).
(1)The ward shall obtain a handbook of physical and mental impairment issued by the municipal or county (city) social administration authority, and shall comply with one of the “items listed under the severe physical and mental disease ” in the following table.
Those who have a physical and mental impairment certificate for a specific severity item or above may apply directly without the hospital evaluation to the county or city long-term care management centers where the current resident of the ward.
(2)Ward should go to the designated announcement “Application for foreign family nursing workers evaluation Hospital” for professional assessment, the diagnosis certificate issued by the designated hospital and the medical team shall determine that caregiving is required for 24 hours, and the assessment results are consistent.
After being evaluated by the medical team, the designated hospital will hand over the original "Disease and Disability Diagnostic Certificate" to the employer. The copy and the "Employer's Application for Employment of the Basic Data Transfer Form for Foreign Nursing Workers" will be mailed to the long-term service center of the county and city governments where the ward lives within 2 days.
(3)Ward who meet the re-recruitment exemption (Only applicable to applications for re-employment of foreign workers):
Ward who meet the re-recruitment exemption qualification do not need to go through the professional evaluation. Therefore, the re-recruitment application can be made within 4 months before the expiration of the foreign nursing care worker's employment. The employer can directly apply for a local nursing worker recommended by the long-term service center.
[Important Reminder] When filling in the [Employer's Application for Employment of Foreign nursing job workers' Basic Data Transfer Form], please fill in the name of the employer and the phone number, in which employer can be contact.
[Reminder] The medical evaluation period is within 60 days from the evaluation date of the medical team. Please be sure to apply for a recruitment permit letter within the validity period to avoid invalidation of the medical assessment and reopen the medical certificate. However, if the ward meets the specific physical and mental impairment program or is exempt from the professional evaluation of the medical institution according to the regulations, the application for the recruitment permission letter shall be submitted within 60 days after the completion of the recommendation by the Long-terms service Center.
The Long-Term care center will forward the case to the Ministry of Labor for review if the native caregiver is insufficient after the long-term care center recommended to the employer.
The long-term care center introduces the native caregiver service sources and recommend the native caregiver to the employers, and provide measurement such as the applicant's domestic long-term care service resources and the incentives for hiring a native caregiver attendants. The purpose is to promote native caregiver industry, reduce the dependence of foreign family nursing workers, and the development of our Long-term care system.
After confirming the receipt information, the Long-Term Service Center of county and city government will handle the above services in accordance with the regulations. If there is no local nursing worker available, the application (employer apply for foreign nursing job workers basic data) will be forwarded to Workforce Development Agency under the Ministry of Labor for review.
Check the results of the Long-Term Service Center
After the completion of the Long Term Service Center’s recommendation, the employer (applicant) or family members can initiatively inquire at the Ministry of Labor's Workforce Development Agency for a result of the application for 7th to 10th day, or visit the Ministry of Labor's Workforce Development Agency website, go to foreign labor application for progress inquiries.
